Consent to Treat

This document outlines your consent for receiving home infusion nursing services from Magenta Care. Please read carefully before proceeding.

Effective Date: February 1, 2026

Your Agreement

By checking the "AGREE" box for the patient consent on the site, you acknowledge your agreement to the terms outlined in this patient consent. Your ongoing use of nursing services from Magenta Care is considered a legal signature and an ongoing acceptance of these terms.

Please be aware that this patient consent is an additional agreement to the Terms of Service you accept when using our platform. If you do not agree to the terms of this consent, we advise against receiving services through Magenta Care.

About Magenta Care

Magenta Care LLC ("Magenta Care" or "we") is a licensed home health agency headquartered in Washington State that provides specialty infusion nursing services. We employ and contract with qualified registered nurses who deliver home infusion therapy under the direction of prescribing physicians and in coordination with specialty pharmacies.

Magenta Care provides nursing services across multiple states through our licensed nursing staff. Our nurses administer infusion therapies as ordered by your prescribing physician using medications dispensed by specialty pharmacies.

Our Role & Responsibilities

Magenta Care provides nursing services including:

  • Medication administration (infusion therapy)
  • Patient monitoring during and after infusions
  • Patient and caregiver education
  • Care coordination with your prescribing physician and pharmacy

Important: We do not practice medicine, diagnose conditions, or prescribe medications. Your prescribing physician is responsible for directing your medical care and treatment.

By accepting this agreement, you acknowledge that you are entering into a provider-patient relationship with Magenta Care for nursing services. Your relationship with prescribing physicians is separate.

Responsibility & Liability

While Magenta Care provides nursing services, the physicians who prescribe your treatment and pharmacies who dispense your medications are considered third-party beneficiaries of this consent.

Magenta Care is responsible for the nursing care provided by our staff but is not liable for:

  • Medical decisions made by prescribing physicians
  • Products dispensed by pharmacies
  • Accuracy or reliability of third-party laboratory tests

Acknowledgments & Agreements

By receiving nursing services from Magenta Care, you accept and agree to the following:

  • No Guarantees: You acknowledge that there are no guaranteed results associated with receiving infusion therapy or nursing services.
  • Potential Risks: You understand that there are potential risks associated with any infusion therapy. Your nurse will inform you of these potential risks and benefits to allow you to make an informed decision about your care.
  • Communicable Disease Risks: You understand the risks of exposure to communicable diseases when receiving in-person nursing services. By inviting our nurses into your home, you acknowledge an increased risk of exposure. You agree to promptly inform Magenta Care if you or anyone in your household displays symptoms of communicable illness.
  • Complete Medical History: You recognize that some treatments may be inappropriate or unsafe based on your health conditions, medications, or supplements. It is vital to truthfully and accurately disclose all relevant health information to the nurses caring for you.
  • Notice to Female Patients: Some treatments may present risks during pregnancy and breastfeeding. You agree to inform your nurse immediately if you are pregnant, planning to become pregnant, or breastfeeding.
  • HIPAA Privacy Notice: You acknowledge receipt of Magenta Care's HIPAA Notice of Privacy Practices.

Remote Communication

Magenta Care may use phone, text, or video communication for scheduling, patient education, and care coordination. Your prescribing physician may offer telehealth consultations separately. By using our services, you consent to receive communications from Magenta Care regarding your care.
