Please be aware that this patient consent is an additional agreement to the terms of service you accept when booking through our platform. If you do not agree to the terms of this consent, we advise against booking any services through this platform.

As detailed in our Terms of Service, Magenta Care LLC ('Magenta Care' or 'we'), the entity that owns this website and app ('Platform') offering the booking platform and associated services ('Service'), operates solely as the platform and booking service provider. We contract with medical entities and providers who deliver medical services. However, it's important to note that Magenta Care is not a medical entity or provider itself and is not responsible for providing any medical services.

Our platform grants users’ access to medical groups and licensed healthcare providers across the United States and worldwide. These medical groups employ or contract with physicians, as well as allied healthcare professionals, such as nurses and physician assistants, to offer various healthcare services as permitted by their local state or jurisdiction. It's crucial to acknowledge that any services you receive from these medical groups and providers, when booked through the platform, are also subject to this consent agreement. Additionally, the medical groups, providers, labs, and pharmacies are considered third-party beneficiaries of this consent. You further agree that Magenta Care bears no responsibility or liability for the provision of any medical services, and you release Magenta Care, along with its owners, officers, members, managers, employees, and agents, from any and all liability associated with the provision of medical services.

We exclusively schedule the medical services you request and handle billing on behalf of the medical groups and providers. We do not provide medical care services, nor are we involved in the practice of medicine. It's essential to understand that we do not make any determinations regarding your healthcare or provide healthcare services. We do not interfere with the practice of medicine by the medical groups or providers, who are solely responsible for directing your medical care and treatment. By accepting this agreement, you acknowledge that Magenta Care is not a medical corporation or healthcare provider. When using the service, you are not entering into a doctor-patient or other healthcare provider-patient relationship with Magenta Care. However, you may enter such a relationship with a medical group and/or one or more providers when using the service.

Each time you choose to receive medical services provided by the medical groups and providers through our service, you provide consent to these entities to assess and provide medical care to you. You have the option to withdraw this consent at any time by discontinuing your use of medical services from the medical groups and providers booked through this platform.

By booking a medical service, you accept and agree to the following:

  • No Guarantees: You acknowledge that there are no guaranteed results associated with receiving medical services.
  • Potential Risks: You understand that there are potential risks associated with any medical treatment available through this platform. The medical groups and providers will inform you of these potential risks and benefits to allow you to make an informed decision about your care.
  • Consent to Telehealth: You agree to receive medical screening through Telehealth Services and acknowledge the associated risks.
  • COVID-19 Risks: You understand the risks of exposure to COVID-19 or other public health risks when receiving in-person medical services. By inviting providers into your home or workplace, you acknowledge an increased risk of exposure to other individuals. You agree to promptly inform the medical group or provider if you or anyone you are in contact with displays COVID-19 symptoms.
  • Complete Medical History: You recognize that some treatments may be inappropriate or unsafe based on your health conditions, medications, or supplements. Therefore, it is vital to truthfully and accurately disclose all relevant health information to the providers caring for you through the medical groups. Failure to do so may affect your treatment outcome, and there will be no liability on the provider or medical group's part should you fail to provide accurate information.
  • Notice to Female Clients: Some treatments may present risks during pregnancy and breastfeeding. You agree to inform your provider immediately if you are pregnant, planning to become pregnant, or breastfeeding.
  • Acknowledgment of HIPAA Privacy Notice: You acknowledge receipt of the Medical Groups' HIPAA Notice of Privacy Practices.
  • Laboratory Products and Services: Certain healthcare services may require at-home diagnostic tests provided by third-party laboratories. Neither Magenta Care, the medical groups, nor your providers can guarantee the accuracy or reliability of these tests.

Consent to Telehealth Services: This section provides information about Telehealth Services, which may be offered by the Medical Groups. Telehealth involves delivering healthcare services using technology, allowing for diagnosis, treatment, care, follow-up, and patient education without an in-person visit.

Possible Benefits of Telemedicine:

  • Easier and more efficient access to medical care and treatment.
  • Convenient access to care at your preferred times.
  • Interaction with providers without in-office appointments.

Possible Risks of Telemedicine:

  • Providers consulting via telehealth may lack information available during in-person exams, potentially affecting their ability to make decisions.
  • Delays in diagnosis or treatment due to technology failures.
  • Potential adverse effects from medication interactions or allergic reactions due to limited access to medical records.
  • Limitations on prescribing certain medications without in-person examinations due to state laws.
  • Concerns about privacy and security of health information transmitted via the internet.

By using Telehealth Services, you acknowledge and accept these limitations and risks. You understand that Telehealth Services are not intended to replace full medical evaluations or in- person visits with healthcare providers. Your Provider may have limitations in providing treatment or making diagnoses via telemedicine, and you may be advised to seek alternative care.

You acknowledge that the laws protecting privacy and confidentiality of medical information apply to Telehealth Services.
